Suitable students will be invited to a placement interview.

We will match you with a suitable placement opportunity.

Complete required documentation and get ready to begin.

Application Guideline

Applying takes two simple steps.

Step 1: Download the placement form relevant to your placement, complete it, and upload it in the form below.

We have two placement forms. Please download the one that matches your placement:

  • Allied Health Placement: for students placed with our allied health team, including allied health assistance.
    Please note: we currently have no allied health placement opportunities open. You are still welcome to submit your form, and if you choose, we will keep it on file in case a placement becomes available.

  • Disability Support Worker Placement: for students completing rostered shift placements with our support workers.
    Please note: we currently have limited spots for remainder of 2026.

Completing the form

Option 1: Fill it in on a computer (recommended)

  • Open the form on a computer. Phones and tablets may not save your answers.

  • Type your answers, tick the boxes, and sign at the end.

  • When finished, go to File → Print and choose "Save as PDF" or "Microsoft Print to PDF" as the printer.

  • Save it with your name in the filename, e.g. Placement Form - Jane Smith.

Option 2: Print and hand-write

  • Print the form and complete it clearly in pen, then sign at the end.

  • Scan or photograph every page, making sure all writing is clear and readable.

  • Save it with your name in the filename, e.g. Placement Form - Jane Smith.

Then upload your saved form below. Saving it as a PDF locks in your answers so nothing is lost or changed when you send it. Forms sent by email or dropped off at our office will not be accepted.

What happens after you apply

Due to the high volume of applications we receive, we're no longer able to respond to every submission individually. If you haven't heard from us within 14 business days, your application has not progressed on this occasion. Unless you ask us not to, we keep applications on file and may contact you when a role matching your skills and availability opens up.

We appreciate the time you put into applying, and we do read every application.

A note on your answers

Please complete the questionnaire and application in your own words. We do not accept applications where answers have been written by AI tools or copied from other sources, and applications identified as such will be declined.

To be clear, this is not about your level of English. We welcome applicants of all language backgrounds, and simple or imperfect English is completely fine. Plain, honest answers written by you will always be received better than polished answers written by something else.

We ask this because your answers tell us what you actually know and how you actually work. Under the NDIS, participant safety depends on workers genuinely holding the skills and judgement they claim. Answers that don't reflect your own knowledge and experience are treated as misleading information under the declaration below.
